SEND Code of Practice
The SEND Code of Practice: 0–25 years is statutory guidance from the Department for Education and the Department of Health and Social Care. It explains the duties of schools, early years settings, local authorities, and other organisations in identifying and supporting children and young people with Special Educational Needs and Disabilities (SEND).

It also provides important information for parents, carers, and young people about what they can expect from these services.
Key Principles
The SEND Code of Practice is built on four core principles:

Children and young people’s needs are at the centre – they and their families should be involved in decision-making.

Early identification – the earlier needs are identified, the sooner the right support can be put in place.

High-quality provision – education, health, and social care must work together to achieve the best possible outcomes.

Successful preparation for adulthood – support should help young people move confidently into further education, employment, and independent living.
The Graduated Approach
The Code outlines a four-stage process for identifying and meeting SEND needs in school:

Assess – Gather information about the child’s needs.

Plan – Agree on the outcomes to be achieved and the support to be provided.

Do – Put the planned support into action.

Review – Regularly evaluate the impact and make changes if needed.
This cycle is often referred to as "Assess–Plan–Do–Review".
Education, Health and Care Plans (EHCPs)
If a child’s needs cannot be met through the available support, parents, schools, or young people (over 16) can request an EHC needs assessment.
If agreed, the local authority will issue an Education, Health and Care Plan setting out:

Parents, carers, and young people have the right to:

Be fully involved in decisions

Access impartial information, advice, and support

Challenge decisions through mediation or tribunal
