SEND Progress and Monitoring

At Wrockwardine Wood Infant School & Oakengates Nursery Federation, we take a rigorous and proactive approach to monitoring the progress of all pupils, ensuring that our provision for children with Special Educational Needs and Disabilities (SEND) is effective and impactful.


Ongoing Monitoring

  • Class Teachers (and your child's Key Person in nursery) monitor and review the progress of all children, using both formal assessments and ongoing observations tailored to the child’s stage of development.

  • SENDCo, Subject Leaders and Assistant Headteachers scrutinise assessment data to ensure that interventions and provision are having a measurable impact on learning and development.


Reviewing Individual Provision

  • For children on the SEND register, the Class Teacher/Key Person and SENDCo review Individual Provision Maps (IPMs) every half term to check that targets are being met.

  • If additional specialist support is needed, referrals to other professionals are made by the Class Teacher/Key Person in consultation with the SENDCo and parents.


Governance and Oversight

  • The SEND Link Governor meets termly with the SENDCo to review the implementation of SEND policies and to ensure that provision is appropriate and effective across both Wrockwardine Wood Infant School & Oakengates Nursery Federation.


Working with Professionals

  • The Executive Headteacher, Pastoral Lead, SENDCo, and Class Teachers/Key Workers meet regularly with external professionals (e.g., speech and language therapists, educational psychologists, health visitors) to ensure that SEND provision is both appropriate and responsive to changing needs.


Transition and Continuity

  • At the end of each academic year, Class Teachers/Key Workers meet with the next teacher or practitioner in a planned transition meeting to share key information.

  • This includes IPMs, relevant reports, and an individual chronology for each child on the SEND register, detailing professionals involved and the support provided, ensuring a smooth and well-informed transition across settings or into the next phase of education.
